Red Card Information
What is a red card?  A red card is a communication tool the athletic office uses to communicate to coaches that student-athletes have all the required information to participate in athletics at Watertown High School.
 
Before participating in Watertown High School Athletics, it is mandatory that every student athlete and a parent/guardian attend a Red Card meeting.  This meeting is to inform athletes and parents of expectations and answer any questions about participating in WHS sports.  A student and parent/guardian only need to attend one Red Card meeting during their High School career.
 
What things need to be done/completed to obtain a red card?
  1. Student-athletes must attend a red card meeting with a parent
  2. A season fee of $75 must be paid (a family max is currently set at $225)
  3. All the forms listed below must be on file in the athletic office
 
The next red card meeting is set for Monday, July 23rd, 2012, at 6:00 p.m. in the Lecture Hall at Watertown High School.

Watertown High School Athletic Registration
The WHS Athletic Registration Form provides general information to the athletic office and also gives the trainer and/or other medical staff consent to treat each student-athlete in case of emergency or injury. A new form is needed every year.
Physical Form
A Physical Form is required for participation. A new physical is required every 2 years.
Alternate Year Form
The Alternate Year Form is used in place of the physical form on alternate years.
WIAA Eligibility Form
The WIAA Eligibility Form needs to be completed every year. Please read completely so you are informed of WIAA rules and regulations.
Code of Conduct Form
Student-athletes must sign the co-curricular code pledging that they will not violate any aspect of the code. This form will only need to be completed once.
Health Disclosure Information
This form gives permission for the trainer to talk to a physician concerning any medical treatment in regards to the athlete and any medical condtion that may occur. This form need only be completed once.
