Watertown 4 Kids and Jefferson County Head Start have adopted this form to use with children enrolled in Head Start- 4K. This cover sheet is submitted along with all Head Start registration materials. The district, school site, and HS all retain copies. This way families do not need to do 2 seperate applications.
